Yana Health Systems Ltd. (dba Yana Motion lab) Consent, Data Handling & Privacy Statement

PURPOSE & EXPLANATION OF THE MOTION ASSESSMENT

I hereby consent to voluntarily engage in a motion assessment for the purposes of learning about my motion health, research, and product testing. I also certify that I have no medical reason not to engage in physical activity or assessment of my movement.

The assessment I will undergo will be performed within a motion capture area where my movements will be tracked and recorded by eight cameras. I will be asked to perform a sequence of movements each approximately ten seconds in length. I understand that prior to beginning the assessment I may be asked to wear different snug-fitting or contrasting clothing for better performance in the motion studio.

I understand that I am responsible for monitoring my own condition throughout the assessment and, should any unusual symptoms occur, I will cease my participation and inform the Studio Technician of the symptoms. Unusual symptoms include, but are not limited to chest discomfort, nausea, difficulty breathing, and joint or muscle pain/injury.

I understand that during the assessment a Studio Technician will monitor my responses, movement and my expressed feelings of effort. I realize that a true determination of my movement capacity depends on focused effort and that at any time I may terminate the assessment for any reason.

RISK

I acknowledge that it is a precondition to the Company permitting me to engage in a motion assessment that I read, understand and execute this Agreement. I acknowledge that the motion assessment will involve tests of motion that may include jumping, cutting and bounding movements (if applicable) in a controlled environment. I acknowledge that the activities I will be participating in involve inherent risk and dangers that may cause serious injury. I fully understand the risk and dangers associated with any participation in such activities and accept same entirely at my own risk and hereby waive any and all claims which I may have against the Company, and its directors, officers, employees, contractors and agents (collectively, the “Personnel”) and hereby release, indemnify and hold harmless the Company and the Personnel from any and all claims, liabilities, suits, losses, or causes of action for damages or injury or death, including claims for negligence, breach of contract or breach of any statutory or other duty of care by the Company or the Personnel arising out of or related to my participation in the motion assessment. Knowing these risks, it is my desire to proceed with the test as herein indicated.

PRIVACY STATEMENT
